From my friend Muhammad (peace be upon him)

Intention:

"Actions are (judged) by motives (niyyah), so each man will have what he intended. Thus, he whose migration (hijrah) was to Allah and His Messenger, his migration is to Allah and His Messenger; but he whose migration was for some worldly thing he might gain, or for a wife he might marry, his migration is to that for which he migrated." -- [Al-Bukhari & Muslim]

Surah Fatiha in Salah:

Ubadah bin Samit (May Allah be satisfied with him) said: "We were behind the Prophet (PBUH) in the Fajr prayer, and he recited a passage from the Quran, but the recitation became difficult for him. Then when he finished he said, 'Do you recite behind your Imam?' We replied, 'Yes, Messenger of Allah.' Then the Prophet (PBUH) said: 'Do not recite anything behind the Imam except Fatihah-tul-Kitab because who does not include it in his recitation in prayer his prayer is not valid.” -- [Abu Dawud, Tirmizi]

Abu Hurairah (May Allah be satisfied with him) reported that the Messenger of Allah (PBUH) said: "If anyone observes salat in which he does not read Surah Fatihah, it is deficient, it is deficient, it is deficient, and not complete." It was said to Abu Hurairah: "What should we do when we are behind an Imam?" He (Abu Hurairah) replied. "Read it in silence." -- [Muslim]
